INFORMATION COLLECTION
All information is securely faxed by the provider to MTM Specialists when they enroll a patient in our program. This information includes demographics, health information, email, address and phone number. This information is used to contact the patients to set up appointments and to ask pertinent information regarding their chronic conditions that their provider desires to be asked. Once MTM Specialists calls the patient they are asked verbally if they want to opt-in for text reminders. The answer is recorded in our EHR.
INFORMATION SECURITY
All communications are HIPPA protected, through encrypted email, fax and electronic health record systems. All employees of MTM Specialists sign a non-disclosure agreement and are held to the highest standards of information protection. No information will be shared with third parties.
SMS TERMS AND CONDITIONS
Information obtained as part of the SMS consent process and client phone numbers will not be shared with third parties for marketing purposes.
SMS OPT-IN OR PHONE NUMBERS
SMS opt-in or phone numbers collected for the purposes of SMS communication are not shared with any outside parties. We use this information solely to communicate directly with you as part of our services.
TYPES OF SMS COMMUNICATIONS
If you have consented to SMS communications from MTM Specialists you may receive text messages from employees regarding their schedules, appointments or tasks you have asked them to perform. No HIPAA protected information will be shared over SMS communications.
STANDARD SMS MESSAGING DISCLOSURES
Message and data rates may apply. You can “opt-out” at any time by texting “STOP”. For assistance text “HELP” or call us at 850-988-1551 or call your primary care provider. You can expect 0-3 SMS messages a month from MTM Specialists, LLC.
